Use this checklist to audit each driver's qualification file for completeness.
Before the Driver Starts (Pre-Employment)
- [ ] Signed employment application — covers minimum 10 years of employment history, includes 3-year accident and violation history, driver's signature and date
- [ ] Pre-employment MVR — from every state where driver held CDL/CLP in past 3 years
- [ ] Employment verification documentation — records showing you contacted all prior employers from the past 3 years, including drug and alcohol inquiry for DOT-regulated employers
- [ ] DOT medical certificate — current and not expired, copy in file
- [ ] Road test certificate (or valid CDL skills test documentation or prior employer certificate)
- [ ] Pre-employment drug test result — negative result confirmed before safety-sensitive duty begins
- [ ] FMCSA Clearinghouse pre-employment full query — driver electronic consent documented, query result documented
Ongoing Requirements (After Hiring)
- [ ] Annual MVR — obtained and reviewed within past 12 months, review documented
- [ ] Annual certificate of violations — signed by driver listing all traffic violations in past 12 months (or none)
- [ ] Current DOT medical certificate — renewed before expiration, most recent certificate in file
- [ ] Annual Clearinghouse limited query — conducted within the past 12 months, result documented
If Applicable
- [ ] Positive drug/alcohol test documentation — result, Clearinghouse report, SAP evaluation, RTD test, follow-up testing records
- [ ] Return-to-duty documentation — SAP evaluation, completion of treatment, negative RTD test
- [ ] Waiver or exemption documentation — if driver has a federal medical exemption (e.g., vision, diabetes, seizure exemption)

Most Common DQ File Audit Violations
- Missing pre-employment MVR from all states — carriers only pull from the current state
- Expired medical certificate — no tracking system to catch renewals
- Annual MVR not completed — review happened but isn't documented
- Clearinghouse queries not completed — limited queries missed for current drivers
- Incomplete employment application — doesn't cover full 10 years, missing signature
- Employment verification gaps — no documentation of contact attempts for prior employers
- Missing pre-employment drug test documentation — test was done but result not in file
Tips for Staying Organized
- Maintain a DQ file checklist at the front of each driver's file
- Use a tracking spreadsheet or compliance software to monitor expiration dates
- Conduct a self-audit of all files at least annually — before an FMCSA auditor does it for you
