What Is a Driver Qualification File (DQF)?

Every CDL driver at a regulated motor carrier must have a Driver Qualification File. Here's what it contains, how long records must be kept, and what auditors look for.

What Is a Driver Qualification File?

A Driver Qualification File (DQF) is a physical or digital file that motor carriers regulated by the FMCSA must maintain for every CDL driver they employ. It documents that the driver meets the minimum qualification standards required by federal law under 49 CFR Part 391.

The DQF is one of the first things a DOT auditor reviews. Incomplete or missing files are among the most common compliance violations found during FMCSA audits.

Required Documents in a Driver Qualification File

1. Application for Employment

A completed application for employment — signed and dated — that covers a minimum of 10 years of employment history. The application must include the driver's employment history, accident history, and statement of violations.

2. Motor Vehicle Record (MVR) Inquiry

An MVR obtained from each state where the driver held a CDL or commercial learner's permit in the past 3 years — not just the current state. Must be obtained before the driver is placed in service.

3. Annual MVR Review

An MVR must be obtained and reviewed at least annually for each active CDL driver. This annual review must be documented.

4. Pre-Employment Drug Test Results

A record confirming that a DOT pre-employment drug test was conducted and returned a negative result before the driver began safety-sensitive functions.

5. Medical Certificate (DOT Physical)

A copy of the driver's current DOT medical examiner's certificate. The certificate is valid for up to 2 years and must be kept current. If a driver has a medical condition requiring more frequent monitoring, the certificate may be valid for a shorter period.

6. Road Test or Certificate of Violations

Either a road test certificate completed by the carrier, or a certificate from a previous employer stating the driver passed a road test within the past 3 years.

7. Employment History Verification

Documentation confirming that the carrier made a good-faith effort to contact each employer from the past 3 years listed on the application. For DOT employers in the past 3 years, this includes obtaining a drug and alcohol testing history.

8. FMCSA Clearinghouse Query

Documentation of the pre-employment Clearinghouse query and any results. The full query (consent-based) must be conducted before employment; a limited query can be used annually thereafter.

9. Certificate of Violations (Annual)

Each year, the driver must submit a signed list of all traffic convictions from the past 12 months (or a statement that there were none).

Consequences of Non-Compliance

Missing or incomplete DQF documents are among the most cited violations in FMCSA compliance reviews. Each missing document represents a separate violation. Patterns of non-compliance can result in:

  • Unsatisfactory safety ratings
  • Fines and penalties
  • Increased audit frequency
  • In extreme cases, out-of-service orders
